Confidentiality Statement

PAFACOM, Inc. upholds the privacy and confidentiality in services of every individual for whom we provide services. We believe that every human being has the right to be treated with dignity and respect; and to that end, all individual clinical, medical, behavioral and personal information remains confidential.

Clinical records and documentation are kept confidential in accordance with N.J.A.C. 10:41 and Division Circular #30.


HIPAA Statement

In accordance with the Health Insurance Portability and Accountability Act of 1996, (HIPAA), it is the policy of PAFACOM, Inc. to protect the privacy of individual health information, and to ensure that such information is used, filed and disclosed appropriately and in accordance with applicable laws and regulations. The Privacy
Rule applies to any “individually identifiable health information” that is transmitted or maintained by a covered entity. PAFACOM, Inc. must obtain valid written authorization before it may use or disclose a person’s health information, unless the use or disclosure is permissible with notice and opportunity to object.
